FGC’s Senior In-Home Counseling Services Program provides outreach mental health services at no cost to Warren County senior citizens who are homebound and referred to our agency by the Warren County Division of Aging and Disability Services. This program is possible with support from Title III of the Older Americans Act and the and the County of Warren- Department of Human Services. Many of these individuals are socially isolated and unable to access community services. They may be impacted by a number of problems, including depression, anxiety, family conflict and difficulty coping with life changes associated with aging. At times more serious problems are encountered, which may include decreased cognitive functioning, the inability to care for oneself or more serious mental illness.
Services provided by the program include:
- Comprehensive biopsychosocial assessments
- Short-term in-home individual and family counseling
- Psychiatric evaluations
- Competency assessments
- Medication prescribing and management
- Caregiver counseling
- Referral and linkage to community services and resources
- Continued treatment for those consumers in need of long-term services
Evidence-based practices, such as cognitive behavioral therapy, are utilized in this program to provide behavioral health services with a high rate of success. In-home services are provided by our core program staff member, who is dually licensed as a registered nurse (RN) and a licensed professional counselor (LPC).
